For our Patients
We strive to provide you the best customer service environment possible when you visit us for your diagnostic imaging studies. This web site page explains a little about what you can expect, and suggests resources to learn more about individual studies. Here you will find:
- answers to common questions and help in scheduling your appointment, obtaining copies of your medical records, or paying your bill;
- links to authoritative sources on the internet whereyou can research the exam your medical team has recommended for you and see how it will help your healthcare provider to diagnose and treat your condition;
- downloadable forms you can print out and complete before your exam then bring with you to Register for your study rather than having to complete them in our waiting room.
While we have tried to anticipate your questions, we encourage you to call our office and speak to our Customer Service Representatives whenever a person-to-person conversation would be helpful.
Imaging Study Reference:
RadiologyInfo , a joint development of the American College of Radiology (ACR) and the Radiological Society of North America (RSNA), is an extensive web resource that provides information about radiologic procedures, the role of radiology in healthcare, and concise and accurate descriptions of individual radiology imaging studies. The site not only explains studies from a patient’s perspective – giving information about what to expect and how the study is done – but it also has videos and images that provide additional depth.
Downloadable Forms
To view the forms listed below, you will need Adobe Reader. You may download Adobe Reader for free at www.adobe.com. Please bring your completed forms with you to our office at the time of your visit.
If you want to review the preparations for your imaging study, or to save time on the day of your appointment, you might want to print out one or more of the Patient Forms in this section. Information required before you take the exam can include dates or medical history that are more easily available to you at home, and by printing ahead of time the form that will be needed you can fill it out at your leisure and then bring it to your appointment – already completed.
